The Department

\n

Health of Older People provides specialised subacute care for older adults requiring further medical assessment, rehabilitation, and support with complex discharge planning. Our services are designed to promote independence, dignity, and quality of life for patients in later stages of care.

\n

We are currently seeking a committed and experienced Clinical Support and Development Nurse to join our team on Ward ACG—a dynamic unit delivering high-quality, patient-centred care to older adults with complex needs.

\n

Ward AC1

\n

Ward AC1 is a 30-bed unit comprised of a Geriatric Evaluation and Management (GEM) service, focused on subacute rehabilitation for older adults following acute hospitalisation.

\n

Patients on Ward AC1 often present with complex medical, functional, cognitive, and psychosocial needs. The unit provides a supportive and collaborative care environment, where staff work closely within a multidisciplinary team to develop and deliver personalised care plans aimed at maximising recovery and quality of life.

The Role

\n

Every Alfred Care Group nurse must practice in accordance with the Nursing Professional Practice Framework. The Professional Practice Framework defines the elements of nursing practice that improve the care and experience of our patients and aligns with the Patients Come First Strategy. It is comprised of six domains that are crucial to maintain standards, lead change, and add value to healthcare. These domains are Comprehensive Care, Workforce, Leadership, Professional Development, Research and Informatics.

As a clinical nurse, your practice will lead to excellence in patient care. You understand the importance of establishing a relationship between the patient and yourself in order to achieve the best possible outcomes for your patients. You demonstrate the core nursing values of patient centred care, trust and respect in every interaction with patients, carers and colleagues. You are accountable for establishing and evaluating the patient's plan of care in partnership with the patient, their carers and other members of the health care team.

Qualifications and Experience Required

• Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
\n
• Current and satisfactory Police Check
\n
• Current and satisfactory Working With Children Check
\n
