Team Leader-Health, Fitness & Wellbeing

We are seeking a motivated and experienced full-time Health, Fitness & Wellbeing Team Leader to support the day-to-day coordination and delivery of NDIS services in the Illawarra region.

Working under the direction of the Services Lead, Illawarra, you will provide quality guidance to staff, support participants to achieve their individual goals, and oversee the delivery of Individual Support and My Life services. This role combines participant support, team coordination, stakeholder engagement and operational responsibilities to ensure high-quality, person-centered outcomes for people with disability.

What will my role entail?

As the Health, Fitness & Wellbeing Team Leader, you will be responsible to-

• Coordinate day-to-day delivery of Individual Support, including community participation and skill development programs.
• Build strong relationships with participants, families, and stakeholders.
• Interpret NDIS plans and support development of SMART goals.
• Assist with rostering, scheduling, and service coordination.
• Maintain accurate records, reports, and participant progress documentation.
• Ensure compliance with NDIS Quality & Safeguards Standards and organizational policies.
• Support and guide team members to ensure consistent, high-quality service delivery.
• Facilitate participant engagement, including meetings, activities, and transport support.
• Contribute to administrative requirements, including funding evidence and reporting.

About You

We are looking for a positive and proactive leader who is passionate about supporting people with disability to achieve their goals and improve their independence. You will bring:

• Certificate III or Certificate IV in Fitness, or an equivalent qualification relevant to health, fitness and wellbeing.
• Relevant qualifications or experience in Community Services, Disability Services or a related field.
• Demonstrated experience in a similar NDIS support or leadership role.
• Strong communication and interpersonal skills with the ability to build positive relationships with a range of stakeholders.
• Excellent organizational and time management skills with the ability to manage competing priorities.
• Strong administrative and computer skills, including working knowledge of Microsoft Office applications.
• High level of accuracy and attention to detail.
• Working knowledge of the NDIS and Quality & Safeguards framework.
• A genuine commitment to supporting people with disability to achieve their goals and participate fully in their communities.
• Current NSW Driver's License.
• Current First Aid Certificate (or willingness to obtain).
• Current National Police Check, Working with Children Check and NDIS Worker Screening Check (or willingness to obtain).
• Eligibility to work in Australia without sponsorship.
