| | || - | - || Workplace Location | Brisbane - North |**About the role**As a Senior Social Worker, you will play a vital role in delivering advanced, person-centred social work services that support consumers, families and carers across a diverse range of healthcare settings. Working within multidisciplinary teams, you will provide high-level psychosocial assessment, intervention, care coordination and discharge planning to individuals experiencing complex health, disability, ageing, rehabilitation, palliative care and chronic disease management needs.
