Diabetes Educator - Co-Management Service

HARP provides specialist treatment, care planning, education and support to help people with chronic and complex health issues to manage independently and reduce their risk of a hospital admission. The Diabetes Co-Management Service operates as part of the Hospital Admission Risk Program (HARP). The service is available to individuals with unstable diabetes who have been admitted to hospital or presented to the emergency department (ED) during the last 12 months.

The role of the Diabetes Educator is to provide direct clinical care and intervention to people living with unstable diabetes in the community who are at risk of hospital admission. In addition to having advanced knowledge within nursing practice, they apply knowledge of current and interdisciplinary best practice diabetes management to guide client centred clinical decision making, referrals, and care.

Your Impact

• Provide diabetes management and clinical support to people with complex needs.
• Work collaboratively with consumers, families, GPs and RMH specialist teams to coordinate care.
• Provide diabetes education, advice and self-management support to eligible HARP clients.
• Deliver care across clinic, home-based and telehealth settings.
• Contribute to a collaborative multidisciplinary team and support integrated, person-centred care.

About You

You are a Registered Nurse with diabetes education expertise and experience supporting people with complex health and psychosocial needs, as well as:

• Division 1 registration with the Nursing and Midwifery Board of Australia under the Australian Health Practitioner Regulation National Law Act 2009.
• Completion of a Post Graduate Diabetes Educators Course accredited by the Australian Diabetes Educators Association (ADEA) and ADEA Credentialed Diabetes Educator status.
• Experience working in hospital and/or community settings, with an understanding of factors contributing to hospital admissions for people with complex health needs.
• A current Victorian Driver's Licence.
