The Role

Benetas Community Health and Care (CHC) provides diverse services to older clients within their homes across Victoria. These services include individual in-home support through Commonwealth funding (CHSP), Support at Home, Veterans Home Care, respite services and private in-home care services.

Through exceptional customer service and thoughtful advice, our Care Coordinators help our clients to make the best use of their services, enabling them to live at home longer.

• Completing client intake and onboarding processes
• Coordinating services in partnership with clients, families, carers and the Benetas team
• Supporting clients to understand their budgets and maximise the use of available resources
• Promoting independence, choice and informed decision-making through clear and relevant information
• Interpreting and applying policies, procedures and relevant aged care legislation
• Assisting with the development and ongoing review of holistic client support plans
• Advocating for clients and liaising with external service providers where required
• Managing a high volume of calls while maintaining accurate client records and documentation
• Supporting compliance requirements under the Aged Care Act and relevant service standards

About You

• A genuine passion for supporting older people to live their best lives
• Strong communication and problem-solving skills
• Demonstrated administration skills and methods for staying organized
• The ability to work both independently and collaboratively in a team
• Intermediate computer skills and ability to navigate systems
• Respect for diversity and the ability to build trust with clients and their families
