What you will own

• Client relationships. Be the primary point of contact for clients and families, from referral and assessment through to ongoing reviews.
• Care planning. Conduct comprehensive in-home assessments and build personalised plans around each client's strengths, priorities, and goals.
• Budgets and agreements. Develop individual budgets, manage service agreements, and monitor package utilisation in line with Support at Home requirements.
• Service coordination. Partner with Scheduling, care workers, allied health professionals, and other stakeholders to deliver seamless support.
• Quality and compliance. Maintain accurate records and ensure services meet the Aged Care Quality Standards and regulatory requirements.
• Better ways of working. Spot opportunities to strengthen the client experience, improve service delivery and support sustainable growth.

You will thrive here if you

• Lead with people. You are warm, perceptive, and genuinely person-centred, with the confidence to hold clear and respectful conversations.
• Know community care. You bring at least one year of relevant case management experience in home-based aged care or community services; deeper experience is highly regarded.
• Understand the detail. You can manage budgets, priorities, documentation, and compliance without losing sight of the person behind the plan.
• Stay calm and find a way forward. You are organised, digitally capable and comfortable making decisions in a changing environment.
• Bring relevant training. A diploma or tertiary qualification in aged care, community services, disability, social work, nursing, allied health, or a related field is highly regarded. A nursing background is desirable.
• Can travel locally. You hold a valid Victorian Driver Licence.
• NDIS Worker Screening Check. You have or are willing to obtain the required NDISWSC clearance.
