About The Role..

Are you ready to make a real difference in people’s lives? As a Senior Social Worker and Case Manager in our Brain Injury Unit (BIU), you’ll play a vital role in supporting clients through their recovery journey. You’ll provide evidence-based clinical care, guide rehabilitation case management, and ensure smooth and well-supported discharges back into the community.

Working within a passionate multidisciplinary team, you’ll have the chance to advocate, problem-solve, and create meaningful change for clients and their families every day. If you’re looking for a role where your skills and compassion truly matter

Key Responsibilities Include

• Counselling to assist clients and their family/carers to deal with the emotional impact of injury, illness and disability.
• To undertake case planning, liaison, advocacy and referral to maximise access to appropriate community services.
• Liaise with multi-disciplinary team colleagues to optimise client care.
• Contribute to the ongoing development of the social work and case management services.
• This role fulfills both the Social Work and Case Management aspects of service delivery. Sound understanding of the iCare (Workers Compensation and Lifetime Care Schemes) is essential.

About You..

We seek a self-motivated and friendly individual with:

Essential Criteria

• Tertiary qualifications in Social Work, eligibility for membership of the Australian Association of Social Workers and commitment to the AASW Code of Ethics.
• Experience and clinical skills - assessment, counselling, advocacy, referral to community services and discharge planning in a health/rehabilitation setting.
• Experience working in a health setting with adults with a physical or cognitive disability.
• Demonstrated competence in communication skills - written and verbal, negotiation, conflict resolution.
• Demonstrated commitment to participation in supervision and professional development.
• Computer skills and demonstrated competency.
• Sound understanding of the iCare (Workers Compensation and Lifetime Care Schemes).
