Job Description

\n

The Opportunity
\n As a Care Partner within our Support at Home program, you will support older people to remain living safely, independently and well in their own homes. Working closely with clients, carers and service providers, you'll deliver high-quality care management and coordinate services that enable positive ageing outcomes.

\n

Responsibilities include but are not limited to:

\n

• Manage a caseload of clients receiving Support at Home services, delivering high-quality, person-centred care management.
\n
• Conduct aged care assessments, develop care plans and complete regular reviews to ensure services continue to meet client needs.
\n
• Support clients and their carers to achieve their goals, maintain independence and make informed decisions about their care and wellbeing.
\n
• Monitor and manage individual client budgets to maximise available funding and support outcomes.
\n
• Coordinate in-home and community-based services with internal teams and external providers.
\n
• Maintain accurate documentation and ensure compliance with aged care legislation, quality standards and funding requirements.
\n
• Build trusted relationships with clients, their families, carers and community stakeholders.
\n
• Travel within the community to conduct in-home client visits, reviews and care planning discussions.

Desired Skills and Experience

\n

About You:

\n

\n
• Minimum 3 years' experience working as a care partner or case manager withinaged care
\n
• Relevant experience in aged care, community care, care management, or a similar client-facing role.
\n
• A tertiary qualification in Social Work, Allied Health, Nursing, Care Management or a related field.
\n
• Sound knowledge of aged care legislation, quality standards, and the Support at Home program.
\n
• Full drivers license and own car (miles will be reimbursed)
\n
• Experience delivering person-centred care and managing client budgets.
\n
• Strong communication and stakeholder engagement skills.
\n
• Ability to manage a caseload, maintain accurate records, and work collaboratively.
\n
• Experience building community networks and partnerships.
\n
• Understanding of CALD and/or Aboriginal and Torres Strait Islander communities.
\n
• Proficient in Microsoft Office, including Word and Excel.
\n
• Experience working in community health, public health, or a community-based setting.
\n
• Ability to travel within the community to support clients.
\n
• Experience working within ageing, community health, or home care programs.
\n
• (Desirable) Ability to speak a second language.
\n
