About the Role

As a Youth Homelessness Case Manager, you will provide holistic, client-centred support to young people aged 15 to 25 who are experiencing or at risk of homelessness. Working across the Brimbank, Melton and Yarra communities, you will deliver case management, outreach, housing support and advocacy, helping young people and their families build stability, wellbeing and positive futures through tailored support and strong community partnerships.

Key Responsibilities

• Provide intensive, client-centred case management, outreach and housing support to young people aged 15-25 who are experiencing or at risk of homelessness.
• Develop tailored support plans that strengthen housing stability, wellbeing, independence and positive outcomes for young people and their families.
• Advocate for clients and coordinate referrals with housing access points, specialist support services and community agencies across Melbourne's West and North.
• Build and maintain strong partnerships with internal and external stakeholders to deliver holistic, collaborative support.
• Manage a caseload, maintain accurate client records and contribute to service reporting in line with program requirements and quality standards.

About You

• Qualifications: Tertiary qualification in Youth Work, Social Work, Community Services or a related discipline.
• Experience: Demonstrated experience supporting young people experiencing homelessness, housing insecurity or other complex needs, with a strong understanding of the homelessness service system.
• Mandatory Requirements: Current Working with Children Check, National Police Check, right to work in Australia and the ability to meet all pre-employment requirements.
• Desirable Skills: Strong case management, advocacy, stakeholder engagement, communication and problem-solving skills, with the ability to work collaboratively across service networks.
• Attributes: Compassionate, resilient and adaptable, with a commitment to client-centred practice, social justice and achieving positive outcomes for young people and families.
• Salary packaging - up to $15,900 in tax-free pay per year, plus entertainment up to $2650
• 4 days gifted leave at Christmas and Easter (pro rata)
