Home Care Coordinator

The Home Care Coordinator will be responsible for the effective coordination and delivery of home care services across Support at Home, Commonwealth Home Support Programme (CHSP) and Brokerage services. The successful candidate will work closely with clients, families, support workers and external service providers to ensure high-quality, person-centred care.

Key responsibilities

• Coordinate and oversee the delivery of Support at Home, CHSP and Brokerage services.
• Manage client services in accordance with individual needs, care plans, budgets and funding requirements.
• Coordinate client intake, assessments, service commencement and ongoing reviews.
• Develop and maintain client schedules and ensure services are appropriately staffed.
• Coordinate and manage staff rosters, shift changes, leave and replacement arrangements.
• Match support workers with clients based on their needs, preferences, skills and availability.
• Liaise with clients, families, support workers, health professionals and other service providers.
• Coordinate contractual arrangements, including engaging Associated Providers, arranging services and monitoring service delivery.
• Monitor client services to ensure they are delivered appropriately, within agreed budgets and in line with client requirements.
• Monitor client budgets, service utilisation and expenditure and identify any issues or variances.

About you

• Highly organised with excellent time-management and administration skills.
• Experienced in home care coordination, aged care, community care or a similar role.
• Knowledgeable about Support at Home and/or CHSP requirements and service delivery.
• Experienced in coordinating Brokerage services and external providers.
• Confident managing rosters, client schedules and changing priorities.
• Compassionate and committed to delivering person-centred care.
• A strong communicator who can build positive relationships with clients, families, staff and external providers.
• Professional, reliable and detail oriented.
• Able to work independently and as part of a team.
• Confident using client management, rostering and administrative systems.
