Our services include general practice (GP), nursing, allied health, dental, counselling and psychology, gambling support, alcohol and drug treatment, migrant and refugee settlement services, social connection, carer support services and National Disability Insurance Scheme (NDIS) services in partnership with the NDIA.

As part of our Local Area Coordination Service, you will support people with disability, their families and carers to navigate the NDIS, build capacity, pursue their goals and connect with supports within their community. You'll join a supportive team environment with comprehensive training and ongoing professional development to help you succeed.

What you will be doing in this role \n

Reporting to the Area Manager NDIS LAC Service, you will work directly with NDIS participants, families, carers, community organisations and mainstream services to help people access supports and achieve their goals.

Your work will include: \n

• Supporting participants through pre-planning, planning, implementation and plan reassessments
\n
• Assisting people with disability to connect with community and mainstream supports
\n
• Working collaboratively with the NDIA and key stakeholders to achieve positive outcomes
\n
• Promoting inclusion, accessibility and community awareness of the NDIS
\n
• Providing participant-centred support through face-to-face, phone and outreach services
\n
• Maintaining accurate records and meeting service delivery outcomes and performance measures
\n

What we are looking for \n

\n
• Experience working with people with disability and/or lived experience of disability
\n
• Strong communication, relationship-building and stakeholder engagement skills
\n
• Excellent organisation, time management and problem-solving abilities
\n
• Ability to work within a fast-paced environment while maintaining quality outcomes
\n
• Intermediate computer skills and experience using digital systems and databases
\n
• Current Victorian Driver Licence
\n
• Current Working with Children Check (Employee) or willingness to obtain
\n
• Current NDIS Worker Screening Check or willingness to obtain
\n
• Qualifications in disability, community services or a related field will be highly regarded but are not essential.
\n
