Lifestyle Coordinator/ Assistant – Aged Care

CroCare is a not-for-profit, community-focused residential aged care home in Geelong. As an ethnospecific aged care provider with strong connections to the Croatian community, culture, language, traditions and meaningful relationships are an important part of life at our home.

We are currently looking for an experienced Lifestyle Coordinator and/or Lifestyle Assistant to join our Lifestyle team.

\n

Previous experience delivering lifestyle and engagement programs in residential aged care is essential. Croatian language skills are highly regarded, but are not essential for the right candidate.

\n

About the roles

\n

Depending on your experience and qualifications, you may join us as a Lifestyle Coordinator or Lifestyle Assistant.

\n

As part of our Lifestyle team, you will help create and deliver meaningful programs that reflect each resident's interests, abilities, background, culture and individual preferences.

\n

Responsibilities may include:

\n

\n
• Planning and delivering individual and group lifestyle and leisure activities
\n
• Supporting residents to maintain interests, relationships, independence and community connections
\n
• Supporting residents living with dementia through meaningful, person-centred engagement
\n
• Completing lifestyle assessments, care planning, progress notes and participation documentation
\n
• Coordinating or supporting cultural celebrations, entertainers, volunteers, outings and community activities
\n
• Seeking resident feedback and adapting programs to residents' changing interests and needs
\n
• Working closely with care, clinical, hospitality and other teams to support residents holistically
\n
• Contributing to a welcoming environment where residents have genuine choice about how they spend their day
\n

The Coordinator role will also have responsibility for planning and overseeing the broader Lifestyle program, coordinating the Lifestyle team and ensuring lifestyle documentation and programs are maintained to the required standard.

\n

About you

\n

We are looking for people who genuinely enjoy working with older people and understand that lifestyle in aged care is much more than running activities.

\n

You will have:

\n

\n
• Previous lifestyle experience in residential aged care – essential
\n
• Certificate IV in Leisure and Health or a relevant qualification, particularly for the Coordinator role
\n
• Experience supporting people living with dementia
\n
• A good understanding of person-centred care, resident choice and dignity
\n
• Strong communication and relationship-building skills
\n
• Confidence completing lifestyle documentation and using electronic systems
\n
• Good organisation skills and the ability to work both independently and as part of a team
\n
• A practical, hands-on approach and willingness to actively engage with residents
\n

\n

Croatian speaking or an understanding of the Croatian language is highly desirable, particularly given our ethnospecific environment and the benefit this provides when engaging with residents who prefer to communicate in Croatian.
