Out of Hospital Care - Home Care Worker

Community Services #1 is seeking a Casual Out of Hospital Home Care Worker to deliver high-quality, person-centred care that supports independence and wellbeing.

About the role:

Provide home care services aligned to Out of Hospital Care goals, including domestic assistance, personal care, support services that assist recovery and independence.

Assist participants during the end-of-life stages of care.

Apply duty of care and safe work practices in client homes, and identify, document, and elevate OHC escalation triggers promptly.

About you:

Certificate III in Individual Support (Ageing) or equivalent, relevant experience delivering home care/aged care support (domestic assistance and/or personal care) and 3+ months aged carer experience.

Experience supporting clients with diverse cultural, emotional and health needs in community or out-of-hospital settings.

Ability to follow care plans and monitor client wellbeing.

Casual role. Remuneration aligned to SCHADS Award - Home Care Employee Level 2 and 3 (casual), plus super. Supportive team environment, supervision, and professional development opportunities.

Requirements:

Current driver’s licence and ability to obtain/maintain required pre-employment checks and certifications (including National Police Check, Working with Children Check and First Aid), plus mandatory training as required.
