What you'll be doing
\n As the Transition and Youth Health Project Officer, you will lead the implementation of the WSLHD Transition Care Strategy and provide strategic, project and operational support to Youth Health Services. This role offers an exciting opportunity to influence healthcare transformation and improve the experience of young people with chronic conditions as they transition between healthcare services.

You will:

\n

• Develop, implement and evaluate transition care strategies that improve healthcare experiences and outcomes for young people
\n
• Coordinate projects, initiatives and stakeholder engagement activities that support integrated and connected care across paediatric and adult services
\n
• Analyse data, gather consumer and clinician insights, and translate findings into service improvements and innovative solutions
\n
• Lead the facilitation and administration of the Western Sydney Youth Council, ensuring meaningful consumer co-design across WSLHD
\n
• Build strong partnerships, prepare reports and briefs, and support committees, education opportunities and strategic initiatives
\n

\n

License: Drivers License valid in NSW and willingness to use for work purposes

\n

Selection Criteria:
\n Bachelor level qualification in a relevant health discipline and at least 3 years post‐graduation experience

\n

\n
• Demonstrated successful track record in project management including change management and implementation methodology, reporting, monitoring and evaluation
\n
• Demonstrated knowledge and understanding of the Transition of Care of young people with chronic conditions in WSLHD and SCHN and the patient context
\n
• Demonstrated ability to collaborate and work in partnership with multiple stakeholders, both internal and external to the organisation
\n
• Demonstrated time management skills
\n
• Demonstrated high level communication skills, written and verbal, negotiation, report writing and research skills and an understanding in using healthcare database systems and data platforms
\n
• Understanding of youth health issues and vulnerable youth populations.
