CHRH is seeking a passionate and motivated Care Partnerto join our growing Care at Home team. In this rewarding role, you will work closely with older people and their families to coordinate services, support goal achievement, and deliver a positive, person-centred care experience that enables clients to remain living safely and independently at home.

As a trusted local health service and registered Support at Home provider, CHRH offers the opportunity to work within an integrated care model alongside nursing, allied health and community support services. Multiple permanent, part time positions available. Hours to be negotiated with sucessful candidates.

Salary: HS5 $48.87 per hour, plus superannuation

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.

It is mandatory for all employees who work within the Health Care Sector have their flu vaccinations. Evidence must be provided prior to commencement with Central Health Rural Health.
