About The Role

\n

As a Registered Nurse, you'll help older Australians stay safe, healthy and independent in their own homes.

\n

This is a highly autonomous role where you'll spend most of your time out in the community, supporting clients across Western Sydney and building genuine relationships with the people you care for. No two days are quite the same, and you'll play an important role in helping clients navigate changing needs while continuing to live safely and confidently at home.

\n

You'll work closely with our Care Partners and broader service delivery team, bringing your clinical expertise, practical judgement and person-centred approach to support great outcomes for clients and their families.

\n

Responsibilities include:

\n

\n
• Delivering nursing care, personal care and clinical support to aged care clients in their homes, ensuring they can continue living safely and independently at home
\n
• Conducting health assessments, reviews and ongoing monitoring
\n
• Providing wound care and other nursing interventions
\n
• Identifying changes in client wellbeing and recommending appropriate supports
\n
• Working closely with Care Partners, families and allied health professionals to achieve great client outcomes
\n
• Maintaining accurate clinical documentation and records
\n
• Supporting quality, compliance and safe service delivery
\n
• Helping clients remain independent and living well at home for longer
\n

Working arrangement:

\n

\n
• Office based in Penrith with travel across the Western Sydney region
\n
• Preference for 0.6 FTE (3 days per week)
\n

\n

\n

About You

\n

You're an experienced Registered Nurse who enjoys working independently and building genuine relationships with clients.

\n

Ideally, you've worked in home care or community-based services and understand the reality of service delivery. Client needs change, schedules shift and priorities can quickly evolve, but you're adaptable, practical and focused on finding solutions.

\n

You bring strong clinical judgement, communicate with warmth and professionalism, and know how to balance autonomy with teamwork. You're comfortable managing your own day while staying connected to a supportive team and contributing wherever needed.

\n

Most importantly, you're compassionate, mature and genuinely motivated by helping older Australians remain safe, independent and living well at home.

\n

You'll also have:

• Experience in aged care, home care or community nursing
\n
• Confidence providing wound care, assessments and ongoing clinical support
\n
• Strong clinical judgement and the ability to work autonomously
\n
• The adaptability to manage changing priorities and day-to-day service delivery challenges
\n
• A collaborative approach and willingness to support the wider team
\n
• A genuine commitment to person-centred care and helping older Australians maintain their independence
\n

Role requirements:

• Current AHPRA registration as a Registered Nurse (Division 1)
\n
• First Aid and CPR certificates
\n
• Driver licence and access to a registered and insured vehicle
\n
• Manual Handling certificate
\n
• NDIS Worker Screening Check
\n
• Working With Children Check
\n
• NDIS Worker Orientation Module
\n
• Australian working rights
