About the role:

\n

Luxe Care is seeking a qualified and experienced Community Worker to join our team in a permanent full-time position. The Community Worker will assess clients' needs, develop appropriate support plans, coordinate referrals and help clients access community, health, welfare and social services. You will work closely with clients, families, Care Managers, healthcare professionals, community organisations and government agencies.

• Interview clients and assess their social, emotional, health, financial and welfare needs.
\n
• Develop, implement, monitor and review individual support plans.
\n
• Provide information, guidance, advocacy and practical assistance to clients and their families.
\n
• Help clients access healthcare, housing, education, employment, welfare, cultural and social support services.
\n
• Coordinate referrals and liaise with community organisations, government agencies and other service providers.
\n
• Support clients to develop life skills, confidence, independence and meaningful community participation.
\n
• Plan and contribute to community engagement, wellbeing and structured support activities
\n
• Monitor client progress and identify changes, concerns or potential risks.
\n
• Prepare accurate assessments, case notes, referrals, progress reports and service documentation.
\n
• Provide culturally responsive, respectful and person-centred support.
\n
• Comply with privacy, safeguarding, workplace health and safety, infection-control and organisational requirements.

We are looking for people who have:

\n

\n
• Qualification in Aged Care, Disability Support, Individual Support, or Nursing - (Certificate III in Aged Care, Individual Support) or equivalent
\n
• At least one year of relevant post-qualification experience in community services, welfare, disability, aged care or a related environment
\n
• Knowledge of person-centred support practices, community service systems and referral pathways
\n
• Expeience assessing client needs and developing or reviewing support plans
\n
• Strong communication, advocacy, problem-solving and interpersonal skills
\n
• Ability to prepare professional case notes, reports, referrals and support documentation.
\n
• A current Australian driver licence and access to reliable transport
\n
• At least 12 months qualified experience in Aged Care / Home Care, ideally in acute setting, residential or community
\n
• A passion for caring for people and delivering a high standard of care
\n
• Excellent communication, customer service and interpersonal skills
\n
• Exceptional organisational and problem-solving skills
\n
• Bilingual or multilingual communication skills are highly regarded.
\n
• Full Working Rights, Valid driver's license and access to a reliable vehicle
\n
• NDIS Worker Screening Check (required if successful)
\n
• Level 2 First Aid Certificate- (required if successful)
\n
• Working with Children Check (WWCC)- (required if successful)
