Success Care Provider Pty Ltd is an NDIS service provider based in Craigieburn, in Melbourne's north, that delivers person-centred disability support to participants in their own homes, in supported and shared living arrangements, and in the community. Its services cover high intensity daily personal activities, help with daily personal activities, support through life stage transitions, community nursing care, daily tasks and shared living support, building daily living and life skills, and community and social participation. The business supports participants with a wide range of disabilities and complex care needs. Its team of around 20 staff is guided by a commitment to independence, safety, dignity and inclusion, with each participant treated as an individual whose goals and preferences shape the support they receive.

About the Position

The Personal Care Assistant is a new full-time role on Success Care Provider's frontline support team. The role provides hands-on personal care and daily living support to NDIS participants across home, shared living and community settings. It is central to the business's high intensity and daily personal activity services. The Personal Care Assistant makes sure participants receive safe, consistent, dignified care that follows their support plans. They also act as the link between participants, the care team and allied health therapists by observing, recording and reporting changes in each participant's condition. The position reports to the management team and includes access to specialised training when required.

Key Responsibilities

• Help participants with personal care, including showering, bathing, toileting, grooming, dressing and eating, in a way that respects their privacy, dignity and preferences.
• Help participants with mobility, transfers and positioning, using safe manual handling techniques and mobility aids and equipment.
• Support participants' communication needs, including the use of communication aids and strategies set out in their support plans.
• Take part in planning individual care and support, contributing observations and participant feedback to care planning and review.
• Follow therapy and behaviour support plans, including interventions for participants living with dementia or behaviours of concern.
• Observe and monitor participants' physical, emotional and behavioural wellbeing, and promptly report and document any changes in condition, incidents or concerns.
• Receive complaints about care, record them and pass them on to management in line with the organisation's feedback and complaints procedures.
• Help participants with rehabilitation exercises and basic treatments as directed by health professionals.
• Help participants take their medication in line with their medication plans and the organisation's medication procedures, and record it accurately.
• Provide direct support to therapists and community nursing staff during therapy sessions and care activities.
• Keep accurate, timely progress notes, shift reports and incident records.
• Follow NDIS Practice Standards, the NDIS Code of Conduct, and the organisation's work health and safety, infection control and privacy policies.

Requirements

• Australian citizens and permanent residents are encouraged to apply.
• AQF Certificate II or III in Individual Support, Disability, Ageing Support or a related field.
• At least one year of relevant experience in personal care, disability support or a related care setting.
• Sound knowledge of safe manual handling, infection control and help with medication in line with care plans.
• Ability to follow individual support plans, therapy plans and behaviour support strategies.
• Clear, accurate record keeping, including progress notes and incident reports, with basic computer skills for digital care records.
• Understanding of the NDIS Practice Standards and Code of Conduct, and a current NDIS Worker Screening Check or the ability to obtain one.
• Respectful, patient communication with participants, families, co-workers and health professionals.
• Strong observation and problem-solving skills, with the judgement to recognise and report changes in a participant's condition.
• Reliable, able to work on your own in participants' homes and in the community, and able to work as part of a team.
• Available to work a 38-hour roster, which may include a mix of shifts.
