About the role:

The successful applicant will demonstrate advanced clinical assessment and formulation skills, sound risk assessment and management capabilities, and a commitment to delivering recovery-oriented, person-centred care. They will possess the ability to work collaboratively with consumers, families, carers, community partners, and external stakeholders to support positive mental health and wellbeing outcomes.

As a Community Clinician, you will play a pivotal role in the coordination and delivery of comprehensive community-based mental health care. This includes providing clinical case management, conducting holistic assessments, developing collaborative care plans, facilitating therapeutic interventions, and supporting individuals to access appropriate services and resources within their local community.

The role requires effective communication and partnership with referrers, multidisciplinary treating teams, primary care providers, and community agencies to ensure seamless care transitions and continuity of support. The clinician will foster family-inclusive and systems-informed practice, supporting consumers, families, and carers throughout their recovery journey while promoting safety, wellbeing, and meaningful community participation.

Located in Rushworth, Waranga Health is a 36-bed aged-care facility. The health service includes a four-bed acute care unit, a district nursing service, outpatient ambulatory consulting services and a dedicated palliative care suite and respite bed. Care and treatment are provided 24 hours a day by a talented multidisciplinary team working across a range of medical, nursing and allied health services.
