Practice Nurse | Chronic Condition Management | Rockhampton

About the Role
We’re seeking a Registered or Enrolled Nurse to join our Rockhampton Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.
Key Responsibilities
- Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
- Conduct health assessments and promote preventative care
- Collaborate with GPs and multidisciplinary teams, including referral coordination
- Manage administrative tasks related to CCM appointments
- Coach patients on sustainable health behaviour changes.
- Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
- Work with the practice team to grow the CCM program.
About You
To be successful in this role, you will have:
- Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
- Prior experience in Chronic Condition Management preferred
- Strong communication and organisational skills
