JOB DESCRIPTION
Job Description
- Provide person-centered care and support to residents, assisting them to maintain their independence, dignity, wellbeing and quality of life.
- Provide person-centered care and support according to individual care plans.
- Assist residents with activities of daily living, including personal care, mobility, dressing, grooming and meal-related support.
- Support residents to maintain independence and participate in daily activities.
- Observe residents' physical, emotional and behavioural wellbeing and report relevant changes to appropriate senior staff.
- Work collaboratively with registered nurses, healthcare professionals, families and other support staff.
- Assist with implementing individual care and support plans.
- Maintain accurate progress notes, care records and other required documentation.
- Support residents with social, recreational and community participation activities.
- Promote residents' dignity, privacy, choices, cultural needs and personal preferences.
- Respond appropriately to incidents, emergencies and changes in residents' support needs.
- Follow workplace health and safety, infection-control and organisational policies and procedures.
- Maintain professional and respectful relationships with residents, families and colleagues.
About the Candidate
- A relevant qualification in community services, individual support, disability, aged care or a related field.
- Relevant experience providing residential, disability, aged-care or community-based support.
- Demonstrated ability to provide person-centered care and assistance with daily living activities.
- Good communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary care team.
- Understanding of dignity, privacy, choice, independence and duty of care.
- Ability to maintain accurate care documentation and follow individual support plans.
- Current police clearance and other mandatory checks applicable to the position.
- Australian driver's license, where required for the duties.
- At least one year of full-time relevant work experience is required for this role.
- Minimum of Diploma in Community Service, Disability, Aged Care or Individual Support.
