We offer attractive remuneration packages and generous leave entitlements.
- Salary range: $128,169 to $137,918 p.a. (HP4)
- Salary range: $114,532 to $122,643 p.a. (NG6-1)
- 4 to 6 weeks annual leave with 14%-17.5% loading
- 12.75% employer contribution to superannuation
- Plus Salary Sacrificing options may be available
About your opportunity:
To provide clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Aged Care Assessors work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.
To provide advisory clinical support, education and information to staff, service providers, external/internal stakeholders, clients and carers.
Requirements:
- Mandatory qualifications and proof of registration (if applicable).
- Travel may be required for this position.
- Possession of class C drivers' licence.
- Minimum of one year demonstrated experience directly delivering services in aged care settings and/or to aged persons.
- This is a Vaccine Preventable Disease (VPD) risk role.
- Probation period may apply.
- Pre-employment checks will be conducted.
