Aged Care Intake & Care Coordination Specialist

Western District Health Service in Hamilton, Victoria, is seeking a compassionate Aged Care Services Intake Officer to coordinate referrals, assess needs and develop care plans for the Support at Home, CHSP and HACC-PYP programs.
