Job description

DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients' team on a long-term assignment (up to 24 months). This role is full-time hours (Mon - Fri) and will be based in Winnellie, NT .

The position of Clinical Care Coordinator involves working collaboratively with clients and internal and external stakeholders to improve the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members, and / or their families.

The Clinical Care Coordinator will adopt a strengths-based, recovery approach to support vulnerable and / or at-risk clients, enhancing their wellbeing and community participation through community engagement, collaborative recovery work, case coordination, and complex case management.

The role requires you to :

  • Liaise and form partnerships with various internal and external stakeholders
  • Provide high-quality, evidence-informed clinical services including intake, assessment, referral, complex case management, and case coordination
  • Maintain clinical records, collate and analyze data from client records, and prepare reports as required
  • Work systemically with key stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and families
  • Participate in community development and service promotion activities
  • Contribute to the development and delivery of group programs

About you :

  • Degree-qualified Registered Mental Health Nurse, Psychologist, Social Worker, or Occupational Therapist with two years of Mental Health experience
  • Full registration with AHPRA or AASW
  • Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card)
  • An Australian Citizen and willing to undertake a National Police Clearance
  • In return you will receive :

  • An attractive hourly rate of $65 per hour + super
  • Monday to Friday, 37.5 hours per week
  • Extensive training and ongoing support & development
  • 24-month contract with potential extensions
