Take on an influential leadership role in coordinating and improving patient care for those experiencing mental health and addiction challenges. As a Nurse Navigator within Metro South Addiction and Mental Health Services, you’ll work collaboratively across teams and services to ensure patients receive safe, connected, and person‑centred care throughout their recovery journey!

About Your Role

You Will

  • Deliver advanced nursing practice focused on patient‑centred, coordinated care.
  • Facilitate seamless transitions across acute, community, and specialist services.
  • Support patients, carers, and families to improve self‑management and health literacy.
  • Apply evidence‑based practice and advanced clinical judgement in decision‑making.
  • Collaborate across multidisciplinary teams and external agencies to enhance continuity of care.
  • Lead initiatives that improve service integration, safety, and patient experience.
  • Contribute to education, research, and quality improvement projects that inform best practice.

About You

You are an experienced, dynamic and compassionate nursing professional with advanced skills in mental health and addiction care.

You Will Bring

  • Extensive experience (5+ years) in mental health or addictions clinical care.
  • Demonstrated ability to coordinate complex care and navigate multidisciplinary systems.
  • Strong communication, advocacy and relationship‑building skills.
  • Advanced clinical reasoning and decision‑making in high‑acuity or complex settings.
  • Postgraduate qualifications or evidence of enrolment in a relevant program (highly regarded).
  • Credentialing as a Mental Health Nurse or Drug and Alcohol Nurse (highly desirable).
