As the Positive Ageing Programme Coordinator, you will support customers to increase their connection with the community in alignment with their individual goals to ensure that all program requirements are met as outlined by the Commonwealth Home Support Program (CHSP) grant funding agreement and organisational requirements. You will also be responsible for the delivery of objectives that promote living life to the fullest for older people by delivering and evaluating activities and programs that focus on social inclusion, client choice and wellness and reablement.

Responsibilities

The role includes :

• Delivering positive ageing programs, events, and wellness services.
• Coordinating and evaluating service delivery aligned with CHSP grant agreements.
• Engaging with older community members to ensure social inclusion and client choice.
• Monitoring and reporting program outcomes.
