As the Client Liaison, you will ensure the delivery of high-quality care services in alignment with Ozcare policies, aged care standards, and program guidelines. Your tasks will include :
- Providing information about care services and review / plan needs with new and current services users
- Conduct client intake assessments and personalised care planning to clients and their respective
- Review and adjust care plans based on changing client needs / goals
- Undertake phone assessments based on referrals received for new and existing clients
- Complete all required documentation for intake and assessment protocols using prescribed tools and database
- Promote seamless and positive experiences for clients by providing advice on care planning
Your Attributes
We're looking for a person who has what it takes to support our community care team to deliver exceptional services to our clients. You have :
- Demonstrated ability to assess, plan and evaluate care with clients, family and other persons
- Demonstrated sound understanding of community based services and referral pathways.
- Demonstrated sound computer literacy (use of email, Word, Excel etc) and data entry skills.
- Excellent organisational and time management abilities
- Administrative experience in information sharing, report writing, scheduling and rostering activities
- The ability to communicate effectively via telephone, email, fax, video conferencing and written mail
- The ability to work off site as required

Role Requirements
- Be vaccinated against Influenza & at least 1 dose of Covid-19 vaccine
- Current NDIS Worker Screening Check
- Current National or QLD Police Certificate
- Current First Aid & CPR Certificates or willingness to obtain
- Good computer and mobile device skills
