Case Manager - Disability & Aged Care (80 / 20 Split) – Phillip, ACT | Full-time
About The Role
Lumia Care is seeking an experienced and compassionate Case Manager - Disability & Aged Care to join our ACT team. This role is primarily focused on Disability services (approx. 80%), with a growing portfolio in Aged Care (approx. 20%).
Reporting to the ACT Manager, you will play a pivotal role in delivering high‑quality, person‑centred services to participants across disability and aged care streams. You'll be the primary point of contact for participants and their families — guiding them through onboarding, care planning, funding support, and ongoing reviews to ensure services are safe, responsive, and aligned to individual goals.
This is a hands‑on, relationship‑driven role that blends clinical and operational coordination with stakeholder engagement and service growth.

What You'll Be Doing
Participant & Case Management
Manage an allocated caseload across Disability (NDIS, iCare, private) and Aged CareAct as the first point of contact for new referrals and service enquiriesCoordinate seamless participant onboarding, including funding, consents, service agreements, and care plansDevelop, review, and monitor individualised budgets and service utilisationConduct quarterly care reviews in line with the Participant Journey frameworkImplement the ParticipantCare Program (day one, week one, fortnight, and one‑month check‑ins)Ensure all participants have up‑to‑date documentation and clinically appropriate care plans (including dementia, diabetes, behavioural support, etc.)Participant Experience & Quality
Proactively engage with participants and their families to maintain high satisfactionConduct weekly wellbeing and satisfaction check‑ins with selected participantsReceive, investigate, and manage incidents and complaints in line with organisational policiesDrive service improvements based on participant feedback and satisfaction surveysSupport participant retention and continuity of careService Delivery & Collaboration
Work closely with Service Delivery to match participants with suitably skilled support workersEnsure rosters are accurate, compliant, and aligned with funding and care needsSupport continuity of care during unplanned leave and service changesEnsure services are delivered within approved budgets and funding guidelinesMaintain accurate records across systems including ShiftCare and funding portalsStakeholder Engagement & Growth
Build strong relationships with Support Coordinators, Allied Health professionals, RNs, ACAT teams, brokers, and government agenciesAttend expos, interagency meetings, and networking eventsCollaborate internally to expand service offerings and drive participant growthSupport growth and implementation of Aged Care services in the ACTWhat We're Looking For
Qualifications & Experience
Degree in Health, Nursing, Allied Health, Social Services, Business, or a related disciplineExperience in disability and / or aged care case management or a clinical / operational environmentStrong understanding of NDIS, iCare, and Aged Care funding modelsExperience developing care plans, managing budgets, and coordinating servicesSkills & Attributes
Exceptional communication and interpersonal skillsStrong stakeholder engagement and relationship‑building abilitiesHighly organised with strong attention to detailConfident explaining funding and service options to participants and familiesSolutions‑focused, adaptable, and resilient in a fast‑paced environmentCollaborative team player with the confidence to take ownership of outcomesWhat Success Looks Like in This Role
