Specialist Support Coordination Services (SSCS) provides high-quality, person-centred support coordination for NDIS participants with complex needs across WA. We specialise in Level 3 Specialist Support Coordination, managing cases involving the justice system, hospital or care transitions, mental health challenges, and housing instability. At SSCS, we advocate, plan, and coordinate supports to navigate complex systems and life transitions, making a real impact on participants' lives.

Role Description

This is a full-time hybrid role for a Clinical Case Manager located in Perth, WA, with some work-from-home flexibility. The Clinical Case Manager will be responsible for providing case management services, developing and implementing support plans, coordinating and overseeing care, and advocating for participants. The role involves working closely with individuals with complex clinical needs, including those transitioning from hospitals prison or leaving DCP care.

Qualifications

  • Mental Health and Social Services knowledge (desirable)
  • Excellent communication and interpersonal skills
  • Ability to work both independently and collaboratively
  • Relevant experience in NDIS support coordination is a plus
  • Bachelor's degree in Social Work, Physiotherapy, Speech, Occupational Therapy, Psychology, or a related allied health field
