Join Darling Downs Health as an Aboriginal and Torres Strait Islander Advanced Health Worker – Sexual Health in Toowoomba. You'll play a crucial role in delivering holistic, flexible, and accessible health care to Indigenous patients and their families, working both independently and as part of a collaborative team to make a significant impact on community health. What Your Day Will Look Like :

Your day will be diverse and rewarding, involving direct engagement with patients and their families to develop and monitor case plans, and coordinate clinical care across various specialty areas. You'll conduct home visits, manage case conferences, and provide health education sessions, all while advocating for and empowering the Indigenous community. Working closely with a multidisciplinary team, you'll ensure culturally appropriate, evidence-based care, and actively participate in professional development and cultural events. Additionally, you'll maintain accurate documentation and build relationships with external agencies to enhance service delivery.
