Job Description
My People Solutions are seeking an experienced Support Coordinator for full time or part time (min 24 hours) vacancy with an established Melbourne based NDIS (registered) provider.
The role requires the candidate to have a minimum of at least 2+ years' experience working as a Support Coordinator (or similar role such as NDIS Planner or LAC).
You must have excellent verbal and written communication skills and the ability to quickly engage and gain trust with a diverse group of people. Have the knowledge and experience of NDIS legislation, guidelines & be up to date with the constantly changing application & implementation of NDIS plans, supports & services.
Attention to detail, good time management skills including delivering work to deadlines are essential for all the roles as is the ability to work autonomously & as part of a team and be able to commit fully to the role.
The role is ideal for an experienced Support Coordinator who are seeking the benefits of working independently while still working for a company and who also has the experience & skills to provide capacity building skill services to participant.
Responsibilities & Duties :
Understand each client's NDIS plan and goals, align the coordination of supports & services and build capacity of clients.
Deliver work that meets minimum billable time, be compliant with NDIS requirements & ensure reporting requirements are completed as per NDIS guidelines.
Must have a strong ethical approach to the role and understand compliance with NDIS legislation & guidelines.
Uphold privacy & confidentially at all times, manage & maintain data, records & information proficiently and ensure proper agreements & consents are in place via systems (e.g., Astalty, Brevity Care & Proda).
Use your extensive local knowledge & experience to support participants identify & engage services & supports that are aligned & assist them to achieve their goals.
Answer emails, calls & enquires and actions tasks from participants and stakeholders in a timely manner.
Where part of a team, mentor other team members to grow and develop in their roles and participate in regular supervision sessions to enable your professional development.
Deliver capacity skill building & development for participants, where required, ensuring support is delivered through person-centric frameworks.
Requirements
Qualifications & Experience
2+ years' experience in a Support Coordination NDIS role (or similar e.g., NDIS Planner or LAC) supporting a diverse client base, understanding NDIS legislation & regulation, be able to stay up to date with evolving and changing guidelines and requirements.
Degree level qualification in a relevant field social worker, psychology, allied health, community services or mental health are highly regarded.
Experience of working with and coordinating support for people with complex disabilities, high intensity care as well as candidates with specialist knowledge & experience of working with NDIS participants with mental health and psychosocial disabilities, autism & ADHD, housing, or leaving school, accessing mainstream and government services & supports, will be highly regarded.
Ability to undertake research, deliver information in an easy-to-understand format and build key relationships with a range of diverse stakeholders is critical.
Must have strong ethics and values, be highly organised and be able to manage competing priorities whilst delivering consistently high-quality outcomes for participants and the team.
Able to work autonomously as well as be part of a team and understand the commercial elements of the role including meeting minimum billable hours, invoicing, service agreements and legal responsibilities.
Full competence with computer systems, apps and other technology relevant to the role.
