Job description

The Role :

Within a primary health care framework, the Clinical Nurse Specialist (CNS) - Aged Care Assessment Team

ACAT) Assessor :

  • Provides discipline specific expertise, support, and advice within the multi-disciplinary ACAT Service, acute

and rural hospitals, and other services both within and outside of the regional Tasmanian Health Service.

  • Assesses and identifies the needs of older persons and appropriate younger people with disabilities and their
  • carers’ and facilitate access to appropriate community and residential Commonwealth Government

    Subsidised services, through a multi-disciplinary team approach.

    Details of Appointment

    Fixed term, part time, day worker position working 64 hours per fortnight / , commencing as soon as possible until 18 December 2023.

  • Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.
  • Salary :  $96,967 - $102,473 pro rata, per annum + 11% superannuation + access to salary packaging

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2019
  • Eligibility :

    Successful applicants will be required to meet the essential criteria :

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
  • Current Driver’s Licence
  • Applicants should note the following criteria are desirable :

  • Postgraduate qualifications or proven experience in the assessment and management of clients with dementia
  • The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted :

  • Conviction checks in the following areas :
  • crimes of violence
  • sex related offences
  • serious drug offences
  • crimes involving dishonesty
  • serious traffic offences
  • Identification check
  • Disciplinary action in previous employment check.
