About the role
As a Care Facilitator supporting clients across South East Sydney and the Inner West, you will be part of our BaptistCare At Home team that provides quality care coordination and service planning to help clients remain living in their homes. You’ll work closely with clients and their families to develop individualised care plans, set achievable goals, and connect them with services that enhance their wellbeing.
