Healthy Ageing Liaison Officer

About the role

As the Healthy Ageing Liaison Officer, you will play a vital role in fostering connections between Glen Eira City Council, community organizations, and health services to enhance the quality of life for older adults in our community. You will initiate programs and services that promote healthy ageing, making certain that older residents can access the resources and support they need to live active and meaningful lives. In addition, the role will also be providing support to carers who play a critical role in our community for all age groups.

Other responsibilities include :

• Actively engage with relevant internal and external stakeholders to build partnerships, promote the navigation service, and extend its reach to those who will benefit.
• Build and maintain relationships with service providers to reduce barriers and improve access to funded services.
• Support residents who require personalised information and practical assistance so they can navigate and make informed decisions about their care and support needs.
• Assist with the development, promotion, and implementation of strategies to increase resident participation and reduce social isolation through community based and City of Glen Eira social / active connection options.

About you

You have a strong understanding of the challenges and opportunities associated with ageing, along with a genuine passion for supporting older adults to live healthy, active, and connected lives. You recognise the vital role that caregivers play and are committed to fostering an inclusive and supportive community for all. With excellent communication and interpersonal skills, you engage effectively with stakeholders to drive meaningful, person-centred outcomes.
