    • About the role
    • An exciting opportunity has arisen within our Support Coordination team to engage with our customers to give them choice and control over their supports.

      This role is responsible for working alongside people and their families to plan and connect to supports as they choose. As a Support Coordinator you deliver the best customer experience to the people and families who choose to use Avivo. You will be responsible for the provision of information and advice that assists with the implementation of individual plans. A Support Coordinator works in partnership with participants, their families / loved ones, support organisations and mainstream services.

      This position requires travel within the North-Metro community and will sit part of our UDirect Team. Who are the UDirect Team? The UDirect team supports participants to direct and manage their National Disability Insurance Scheme (NDIS) funding in a way that best suits them.

    • There's no 'typical day' but it may include
    • Provide advice and support to people and families, in a clear and informed way
    • Resolve service delivery issues
    • Develop a comprehensive understanding of the NDIS and its requirements
    • Coordinate support from a range of resources
    • Strengthen peoples and families ability to connect with formal and mainstream supports
    • Assist customers and support organisations to report on progress
    • Develop participants capacity
    • What we need from you
    • An understanding of the NDIS, self direction and flexible support options
    • Ability to connect well with people
    • A well-developed ability to think creatively and develop solutions based on contemporary practice
    • Good attention to detail, problem solving and people-centred decision making skills
    • Well-developed computer skills
    • Excellent communication and interpersonal skills
    • Must hold a valid Driver's Licence as travel is required

    • A salary of $83,049 per annum plus super
    • Option to Salary Package up to $18,550 to reduce taxable income - meaning you can legitimately pay less tax and increase your take-home pay
