Lead a specialised Social Work team in a high-performing clinical service

Use advanced practice skills to support complex stroke and aged-care cases

Influence service development, education and multidisciplinary collaboration

Lead, coordinate and deliver advanced Social Work services in Stroke & Rehabilitation, managing complex cases and driving excellence across Aged Care.

Employment Type : Temporary Part Time (up to 01 / 03 / 2026)

Position Classification : Social Worker Level 3

Remuneration : $58.95 - $60.86 per hour

Hours Per Week : 14

Location : Westmead Hospital

You’ll be part of the Geriatric Medicine Department at Westmead Hospital, one of Australia’s leading tertiary hospitals. Working within the Acute Stroke and Rehabilitation stream, you’ll join an experienced, supportive and collaborative multidisciplinary team dedicated to high-quality, patient-centred care.

What you’ll be doing

As the Social Work Team Leader, you will provide advanced clinical services, team leadership and specialist expertise across Acute Stroke and Rehabilitation. You will operate autonomously, lead complex interventions, and guide staff in delivering high-quality, safe and coordinated patient care. Key responsibilities will be to :

  • Manage and lead Social Work services across the Stroke and Rehabilitation wards
  • Deliver advanced psychosocial assessment, counselling, crisis intervention and advocacy
  • Collaborate with NUMs, medical, nursing and allied health teams to support high-risk and complex cases
  • Provide supervision, education and consultation to staff, students and external stakeholders
  • Support service planning, evaluation, policy implementation and quality improvement initiatives

Qualifications and Registration

  • Accredited Degree in Social Work
  • Registration in line with Award requirements : Eligibility for membership of the Australian Association of Social Work (AASW). For overseas qualifications, evidence of eligibility for AASW must be provided at the time of application.
  • Selection Criteria

  • Demonstrated experience in the delivery of social work services in Aged Care, Stroke and or Rehabilitation, in a hospital or community setting
  • Demonstrated high level skills in psychosocial assessments, adjustment to illness / disability / new diagnosis / abuse of older people / Guardianship matters, NDIS matters, grief and bereavement, accommodation placements and complex familial presentations.
  • Experience in developing and implementing policies and procedures for service improvement and skills in providing training and education
  • Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills, and high-level computer literacy.
  • Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse
  • Ability to initiate, lead, complete and implement quality improvement initiatives and service evaluation processes and support ongoing quality improvement activities with other staff.
  • Demonstrated experience in leadership including experience in staff and / or student supervision.
  • Willingness to participate in the After Hours Crisis On-Call Social Work Service, and Unrestricted NSW Driver’s Licence
