Community Care Case Manager (Fixed Term 12 months)

THE ROLE
Warramunda is seeking an enthusiastic, focused Home Care Packages Case Manager to join our friendly supportive team on a permanent full-time basis 12 month fixed term contract.
Your role will be dedicated to supporting consumers in extending their independence and enhancing their quality of life within their own homes. The role will provide a high level of customer service to our clientele in conjunction liaising with the multidisciplinary team.
As a Case Manager at Warramunda , you’ll play a vital role in supporting older Australians to live independently and confidently in their own homes. You’ll work closely with consumers, their families, and care teams to create tailored care plans that reflect individual goals, preferences, and wellbeing.
You’ll work hand-in-hand with internal and external service providers to coordinate supports, problem-solve challenges, and ensure a seamless experience for every consumer. Responding with empathy and professionalism, you’ll manage enquiries, concerns, and complaints in a timely manner—keeping consumers informed, supported, and confident in the care they receive.
As a passionate advocate for quality care, you’ll also help promote Home Care Packages to prospective consumers, sharing the benefits of our services and supporting people to make informed choices about their care.
