Key Responsibilities
- Accept referrals and engage NDIS participants, their families, carers, and informal supports using trauma-informed and recovery-oriented practice.
- Current NDIS Worker Screening Check is required for engaging with participants.
- Support participants, including those with psychosocial disability, to implement NDIS plans in line with their goals, strengths, and recovery journey.
- Promote choice, control, self-determination, and independence in all interactions.
Participant Engagement, Recovery & Plan Implementation

Identify, source, and coordinate informal, mainstream, and funded supports to meet participant needs, including mental health, psychosocial, and community-based services.Link participants to appropriate service providers, including Spectrum Care where appropriate, ensuring informed choice and conflict-of-interest safeguards.P2 driver's licence is acceptable for providing assistance linking participants to appropriate providers.Coordinate services to ensure continuity of care, risk mitigation, and stability of supports.Provide time-limited (specialist) support coordination in complex or high-risk situations to stabilise supports and build participant capacity and resilience.Psychosocial Recovery & Capacity Building
Support psychosocial recovery by assisting participants to build daily living skills, social connections, confidence, and community participation.Work collaboratively with mental health services, clinicians, and community supports to support recovery outcomes.Assist participants to manage service breakdowns, crises, or transitions through coordinated, recovery-focused responses.Documentation, Reporting & Compliance
Maintain accurate, timely, and compliant participant records, progress notes, and data within approved systems.Ensure documentation reflects recovery-oriented practice, participant goals, and outcomes in line with NDIA requirements.Prepare and submit progress reports, reviews, and other documentation to the NDIA and management as required.Participate in plan reviews and regularly report on progress toward participant outcomes.Planning, Teamwork & Professional Practice
Develop intervention or coordination plans for implementation by Disability and / or Mental Health Support Workers where required.Work collaboratively within a multidisciplinary team while independently managing an allocated caseload.Comply with all Spectrum Care policies, procedures, WHS requirements, and quality standards.Participate in approved professional development and mandatory training.Undertake other duties as reasonably directed, consistent with the scope and responsibilities of the role.1-2 years of experience in disability support or NDIS is required for this Community Services & Development role.
Certificate IV in Disability Support or related field is required at commencement.
Requirements
Qualification in one or more of these courses : Certificate III in Individual Support (Ageing, Home and Community); Certificate III in Individual Support (Disability Services); Certificate III in Community Services; Certificate III in Aboriginal and / or Torres Strait Islander Primary Health Care; Diploma of Community Services.Diploma of Mental Health; Diploma of Counselling.Experience of 1+ years.National Police Check.WWCC; Drivers License.POI (100).NDISWS.Certificate of Covid-19 Vaccine.#J-18808-Ljbffr
