Overview
SHARE + CARE SERVICES is a registered NDIS Disability Service Provider based in Adelaide region. We are seeking experienced support workers who can make a difference by supporting people with disability and / or mental health challenges to achieve their goals and to live an independent life. We provide quality, person-centred support and want you to join us and help our participants get more out of life.
Key Responsibilities
- Support participants to participate in age-appropriate activities and tasks including employment, in home support and community access
- Work in accordance with the medication administration policy, work health and safety procedures, participant’s right's policies and privacy and confidentiality
- Monitor safety and well-being of the participants and report to Client Care Manager
- Provide 1 : 1 and 2 : 1 support in response to individual participants' needs
- Ensure completion of appropriate documentation, including relevant information on supports provided, i.e., incident reporting, medication management documentation, progress notes, ABC data, etc.
- Work with participants, families, and relevant professionals to support the participants individual goals
- In conjunction with other members of staff, assist in developing, implementing, evaluating trainings and behaviour programs to attain identified individual goals (BSP)
- All participants receive a high level of care and support
- Personal care is provided to participants in a caring and supportive way
- Documented evidence demonstrates that policies, procedures and protocols are adhered to
- Records demonstrate that participants have received correct medical assistance, dietary intakes and social and moral support
- Participants' files are updated and reviewed
- Daily documentation completed
- A high level of communication is maintained, including advising of any potential risks to participants and / or the organisation
- Mandatory Induction, WHS, Medication Administration, Infection Control (and many more) trainings are undertaken prior to commencement
- NDIS Code of Conduct is adhered to
Skills + Experience
Prior experience, min 1 year, of working with people with psychosocial needs or lived experience with supporting moderate to severe mental health illnessYou have a passion for supporting people to help live their best lifeYou have the ability to see the potential in people and show initiativeYou are creative and proactive in identifying and responding to changesYou are empathetic and a good listenerYou enjoy working independently and in a team environmentExcellent verbal and written communication skillsYou must be available to work morning, sleepover, weekend, andYou are required to have
