About the role

We are seeking a values-driven Rehab Intake Coordinator who will effectively manage and support residential wait lists and bed allocation for The Salvation Army (TSA) residential rehabilitation programs in Victoria (Bendigo Bridge, Bridgehaven and The Basin). Review and acknowledge all referrals received for the residential rehabilitation programs, ensuring risk and prioritisation requirements are noted and clients are responded to in a timely manner. This position is responsible for providing support to clients who are on waiting lists for residential rehabilitation.

You will successfully

  • All service providers have up to date service and referral pathway information.
  • Responds to referrals in a flexible and timely manner, with referrals acknowledged no longer than 48 business hours
  • Clients incoming to program have safe transport arrangements, including negotiation & flexibility with service providers in consultation with the Program Development Manager
  • Liaise with allied health and other health care professionals to obtain the necessary medical and other healthcare records
  • Ensures that clients have exit and post treatment plans in place
  • Attend weekly Clinical Assessment Team (CAST) meetings and present clients to discuss for admission
  • Review the needs of all accepted participants in line with priority requirements and bed availability to ensure an adequate number of beds are allocated to maintain bed occupancy at all sites
  • Ongoing communication between intake coordinators and residential services regarding admissions
  • Assertive referrals are provided as required e.g. psychological support, family violence counselling, financial counselling / literacy, nutrition and life skills training and other health initiatives to support client recovery where needed
  • Evidence based interventions are delivered in line with client centered treatment that aligns with AOD training and qualifications
  • Case management support is provided to clients in accordance with TSA case management models and standards of practice
  • Clients are assisted to build networks and post treatment support plans that include personal networks, after care, self-help groups and community activities
  • Accurate, up to date client records and data is maintained for all clients in accordance with TSA standards and procedures using TSA information systems
  • Referrals are responded to in a flexible and timely manner
  • Risks to clients, staff and TSA are identified and bought to the attention of the Program development manager as soon as possible
  • Be active in own professional development
  • Attend and participate in supervision and debriefing sessions
  • Attend and participate in regular staff meetings
  • This is a full-time, permanent position located in Basin, VIC. Salary and conditions are in accordance with the SACS VIC Lvl 5 - Social Worker award.

    You will have

  • A minimum diploma level qualification in Community Services, Alcohol and Other Drugs (AOD) or relevant field.
  • Relevant experience in a social service environment, preferably AOD
  • A national criminal history check is required prior to commencement.
  • Victoria Working with Children check is required prior to commencement.
