About the Role
Reporting to the Pastoral Care Manager, you will utilise and share your pastoral expertise, coach staff and volunteers, and support the emotional and spiritual needs and goals of clients, their families, and staff. This is a permanent part‑time role (Monday‑Wednesday) at our Woy residential facility.

Responsibilities
Assess resident / family needs and goals on entry to the service using the HOPE assessment toolDevelop a spiritual care plan and work with residential staff to ensure client / family spiritual needs and goals are acknowledgedParticipate in case management reviews and contribute to the development and evaluation of care plansSupport residents and significant others in an ongoing way and in times of crisis to nurture spiritual developmentRecord and document delivery of pastoral care in progress notes and consumer feedback via Riskman, etc.Lead regular carer support groups and faith‑supporting group activitiesProvide prayerful support to staff and volunteers, distributing relevant material, articles and books as appropriateWork with staff to extend their skills in understanding and meeting the spiritual needs of residents, including cultural safetyPresent pastoral care related training sessions to staff and volunteersOffer Christian perspectives on ethical issues to staff and volunteersProvide staff and volunteers with ongoing emotional / spiritual support and debriefing, especially in times of trauma or crisisAct as a point of contact for internal and external support servicesNetwork with other religious leaders and services, coordinating relevant care, resources and group activitiesStrategically develop tailored resources, ideas and activities to enhance the spiritual and emotional wellbeing of residents and familiesCommunicate and build trust by developing relationships with staff and volunteersLiaise with managers, case managers and attend staff and case management meetings as requiredCollaborate with other Pastoral Care Coordinators, leading in training, mentoring and deploying Pastoral Care VolunteersDevelop skills in emotional support, spiritual assessment, pastoral care and the ability to refer to counselling services if requiredDevelop skills to meet the spiritual needs of people who are cognitively impairedMeet regularly with the Pastoral Care Manager for mentoring and guidanceDeliver staff training in areas related to pastoral care, including boundaries, grief and loss, faith and hope, and other areas identified with key stakeholdersProvide opportunities for staff to understand the Christian values of HammondCareAbout You
Strong understanding and experience with people living with dementia, the impact on carers, and journeys of grief and bereavementStrong personal and active Christian faith and church involvementRelevant theological / pastoral care qualifications and experienceDemonstrated capacity to pastorally care for and enhance the wellbeing of those in needHighly developed communication skills, including listening, compassion and empathyExperience in raising up, leading and supporting a teamStrong administrative and computer skillsOur Culture
