Your new role
This is an exciting time for Sacred Heart Mission (SHM) as we are now working within our bold new strategic plan, Strategy 2030 : Serving more people and more communities. As a part of this we are working towards growing our services for older people so more people can experience better housing, health and social and economic outcomes.
In the newly created role of Clinical Practice Lead (Aged Care), you'll spearhead best-practice, trauma-informed, and person-centred care across our aged care programs, both at Sacred Heart Local (home care) and Sacred Heart Community (residential).

Your day-to-day
Leading the development and implementation of clinical systems and procedures across aged care programs.Providing expert clinical advice and support across the organisation.Designing and delivering targeted staff training, clinical mentoring and reflective practice.Partnering with program managers and quality staff to embed continuous improvement and ensure compliance readiness.Driving clinical governance activities including contributing to monthly Quality Care reporting and accreditation planning.Analysing data from incidents, audits, and quality reports to drive improvements.
We're looking for someone with
Current AHPRA registration - Registered Nurse (Div 1)Extensive clinical practice experience within Aged CareA strong understanding of Aged Care Quality Standards, NDIS Standards, and current reform directions.Proven ability to lead staff training, coaching, and performance improvement.Exceptional communication and collaboration skills with a proactive and solutions-focused mindset.A passion for person-centred care and trauma-informed practices.
