Clinical Care Coordinator - Aged Care - Cairns

Role

As the Clinical Care Coordinator, you will take the lead in overseeing and coordinating clinical care, guiding a dedicated nursing team to deliver the highest standards of care—all with the strong backing of a supportive Care Manager and Facility Manager.

Responsibilities

Provide clinical leadership, support, coaching and mentoring to staffCollaborate with other healthcare professionals to ensure the effective development and delivery of appropriate care plansEnsure compliance with the facility's documentation requirementsMaintain clinical governance, supervise infection control protocols, and oversee medication managementFoster strong relationships with residents, families, and stakeholders to promote optimal health outcomes

Key Candidate Requirements

Tertiary qualification in Nursing with AHPRA RegistrationFull Australian working rightsStrong knowledge of regulatory requirements, care governance, and clinical standards in aged careProven ability to lead teams and manage complex care scenarios in an aged care environmentDemonstrated understanding of AN-ACC assessment processesPrevious experience working within a Residential Aged Care setting in a similar roleCandidate Skills & Attributes
