• About the Role

As the Care Coordinator, you will manage individualised care, support, and services for customers, ensuring their safety, independence, and wellness at home. Collaborate with multidisciplinary teams to deliver clinical care and achieve positive outcomes.

• Duties
• Co-design and implement personalised care plans
• Coordinate services and schedules for consumers
• Monitor budgets and documentation accuracy
• Respond to consumer inquiries and complaints promptly
• Participate in meetings and audits as required
• Skills & Experience
• Experience in aged care coordination or similar role is essential
• Strong understanding of aged care funding guidelines and legislation
• Excellent communication and interpersonal skills
• Ability to work autonomously and within a team environment
• Minimum diploma qualification in Human Services / Health / Community desirable
