The Role
We are on the hunt for a skilled, empathetic and passionate Aged Care, clinical professional to lead our Care team in the role of Care Manager with our passionate team in Gosford.
Care Manager duties include, but are not limited to :
- Leadership and coordination of all aspects of case and clinical services across the facility - including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes
- Support the Facility Manager in the management of facility KPI's including for admissions, occupancy and bond management
- Responsible to manage all aspects of the assessment, documentation, submission and KPI targets for ACFI are achieved within the facility
- Oversee and monitor facility medication management practices and processed
- Support the effective documentation and full clinical team user support of policies, procedures and IT system requirements in the facility
- Coordination of assessment, care planning and review processes within the facility, including for all new residents
- Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families
- Key role in leading the care team to accurately record information in accordance with all organisational and legislative requirements
- Assist in the development of facility team rosters and ensuring optimal skill mix available for delivery of care to residents
- Assist in the management and monitoring of all facility complaints and the associated resolution process
- Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility and ensure a commitment to continuous improvements within the facility

Experience at leading and motivating clinical teams in a senior clinical position is essentialYou will have empathy for our residents and be committed to continually improving the quality of clinical care provided to themExperience and extensive knowledge of clinical assessments, care plans, ACFI and accreditation compliance and documentationEngaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environmentCurrent unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)Post graduate qualifications in aged care and business management would be highly desirableHigh level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirementsA confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomesAbility to manage multiple stakeholders and have a balanced and level headed approach to emergency and stressful situations that ariseWhat we offer :
