About the Role :

As the first point of contact for many new clients and families, you’ll guide people through their aged care journey. You’ll conduct assessments, develop tailored care plans, and coordinate services across a range of programs, ensuring each person receives the support that best meets their needs.

Key Responsibilities :

• Conduct client intakes and assessments (in-home, over the phone, or in-centre)
• Develop, review and update individual care plans in partnership with clients, carers, and health professionals
• Monitor changing client needs and ensure services remain responsive and appropriate
• Maintain accurate, high-quality records and support compliance and audit processes
• Build strong relationships with clients, internal teams, and community stakeholders
• Contribute to service improvements and quality assurance activities
