In this full-time fixed term (end date26 / 12 / 2025) role, you will manage a caseload of Aged Care or NDIS-funded clients.
Your primary responsibilities will include connecting clients with internal and external services, guiding them through their Aged Care or NDIS journey, and helping them achieve their defined and funded goals.Key Responsibilities : Develop and implement service agreements.Address feedback about services and assess service referrals to ensure funding eligibility.Monitor client budgets and costings.Conduct in-home visits to better understand client needs.Ensure compliance with Aged Care and NDIS standards.Experience or qualifications in Disability Services or Aged Care Services, ideally within a similar role.Excellent interpersonal and communication skills.NDIS Worker Screening Check (or willingness to obtain).Current Australian "C" Class Drivers Licence with no restrictions.Exceptional time management and organisational skills to prioritise your workload and meet conflicting deadlines in a busy environment.Administrative skills with the ability to multitask in a dynamic environment.A positive attitude, which makes a huge difference28th May, 2025We are looking for an experienced Client Liaison Coordinator to join our Wilson service centre.
