The Role
We are seeking a compassionate and experienced Coordinator to lead the delivery of our innovative Pause Program, supporting people living with dementia and their carers through tailored respite experiences. This role focuses on enhancing wellbeing by coordinating person-centred care plans and facilitating meaningful respite at various stages of the dementia journey. You’ll work closely with clients and families to create flexible support options that promote living well at home for longer.
This position is offered as a 12-month fixed-term, part-time role, working 75 hours per fortnight . Hours are 8 : 30am to 4 : 30pm , based at our Head Office in Subiaco, with flexibility on which weekdays (Monday to Friday) are worked for the right candidate.

About you
You are a qualified health professional with strong clinical assessment skills and a passion for supporting people living with dementia and their carers. You thrive in collaborative environments, have experience in delivering person-centred care, and can translate program learnings into practical, innovative respite solutions. Your excellent communication, planning, and documentation skills enable you to work effectively with clients, families, and stakeholders to improve wellbeing and support living well at home.
