Based in Northcote, in this role you will provide guidance and leadership around the delivery of our CHSP program, as well as supporting internal resource allocation for our HCP Program. With an emphasis on workforce management, you will provide valuable expertise to a growing and evolving workforce in a supportive leadership environment.
The Program Manager is a member of the home care leadership team across Victoria and Tasmania and will work in conjunction with the Regional Director to lead the development and delivery of Home Care and Respite Services to older people, their families and allies living in the Northern communities of Melbourne.
In this role, you will :
Provide leadership and effectively manage the provision of high quality, compliant and consumer directed services Manage and drive our growing Home Care Program and CHSP Program Develop and implement fee for service approaches and ensure that the programs operate within agreed resource frameworks and financial targets Ensure client assessments are conducted as required and ensure consultation with clients and / or their carers in the development of individual goals to maintain or improve independence About you : You are passionate about what you do and want to make a difference in the lives of our clients, enhancing their self-esteem and empowering them to make choices.

Tertiary qualification in management, business or related discipline and / or experience in operational management in a health / aged care organisation Sound knowledge of aged care and business development Comprehensive knowledge of best practice and experience in assessment, care planning and service delivery to older persons living in the community Experience and expertise in working within and monitoring budgets Experience and expertise in the leadership and development of a cohesive team, to support staff in the delivery of high quality services Understanding and experience in identifying and managing risk in the community aged care Thrive in a team and also appreciate the one to one interactions A commitment to being part of a workplace culture based on the B E S T way of life (Believe, Excel, Support and Trust ) You will need : A current Working With Children's Checkcard Full Australian working rights Current police records check orNDIS (as required) (or be willing to undertake this process) Current and ongoing flu vaccinations Make a positive difference in your community – apply now!
