We specialise in palliative care, rehabilitation, aged and dementia care, mental health services for older people and other related health and aged care services. HammondCare seeks to embed evidence-based best practices in its services and we provide these services through home care, sub-acute hospitals and residential aged care.

About The Role
This Permanent Part-time role (0.6 FTE) for an experienced and skilled Registered Nurse is based within the Community Palliative Care team at Greenwich. You will be a part of a highly dedicated team that provides the best nursing care and advice to patients with palliative care needs.

About You
Current registration with the Australian Health Practitioner Regulation AgencyExperience working in the specialty area of palliative care in an inpatient setting or community is desirableFour years post-graduate experience and working towards a post-graduate palliative care qualification.Evidence of integration of theoretical knowledge and clinical skills, critical thinking, and analysis in your daily practice as a registered nurseAble to work within a multidisciplinary teamEffective written and oral communication skillsCommitment to ongoing professional developmentComputer literacy with email and an electronic patient record systemEffective time management skills and ability to prioritise competing workloads within a complex environment
